Handovers between shifts are one of the most vulnerable moments in disability support. A detail missed during handover can affect a participant’s entire day. Yet many teams still rely on verbal handovers or sticky notes.
When information falls through the cracks, support workers lose context about participant needs and medication changes. For NDIS providers, poor handover practices also create gaps in documentation that become audit issues.
A structured digital handover includes a shift summary, action items, participant updates, medication records, and escalation flags. When handovers are logged in a central system, everyone on the care team has access to the same information.
The most effective approach is to make handover documentation part of the shift closing process. Support workers complete a note before finishing their shift, and the incoming worker reviews it at shift start.
